We are seeking a dedicated and experienced Registered Nurse Case Manager to join our healthcare team. This role is pivotal in coordinating patient care, ensuring effective treatment plans, and facilitating communication among healthcare providers. The ideal candidate will possess a strong clinical background and demonstrate exceptional skills in patient assessment, discharge planning, and case management across various medical settings.

Duties

  • Conduct comprehensive patient assessments to determine care needs and develop individualized care plans.
  • Collaborate with multidisciplinary teams to coordinate patient services and ensure continuity of care.
  • Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
  • Facilitate discharge planning, ensuring that patients transition smoothly from inpatient to outpatient care.
  • Maintain accurate medical records in compliance with HIPAA regulations and organizational policies.
  • Provide education and support to patients and families regarding treatment options, health management, and resources available.
  • Utilize critical thinking skills to evaluate diagnostic information and make informed decisions regarding patient care.
  • Engage in utilization management activities to optimize resource use while maintaining high-quality patient outcomes.

Experience
Candidates should have the following experience or skills:

  • Critical care experience, including ICU or Emergency Room settings.
  • Familiarity with Level I and Level II Trauma Centers.
  • Background in hospital medicine, infectious disease care, geriatrics, behavioral health, pediatrics, or medical-surgical nursing.
  • Experience in outpatient settings or clinics is advantageous.
  • Knowledge of medical terminology, physiology, and vital signs monitoring.
  • Proficiency in case management processes including intake assessments and diagnostic evaluations.
  • Understanding of post-acute care dynamics and managed care principles. This position requires a Registered Nurse license with a strong commitment to providing high-quality patient service and care.
